- School Principal

- Attorney-at-Law
- Credit Union Board/ Committee Member
- Credit Union Employee at supervisory level, employed for more than one (1) year
- Credit Union member for more than two (2) years and in good standing
- Proof of Address (should not be older than six months)
- Utility Bill

- Bank/Credit Card Statement
